Patient Resources

When scheduling your first appointment, we will ask you for a valid cell phone number. This is primarily used as a tool to communicate with you about future appointments, announcements, etc. However, for your first time appointment, it will also be used to send you electronic versions of our new patient forms. There will be no printing necessary. Be sure to complete your forms and provide a valid photo ID for the responsible party PRIOR to your first appointment.

If you are unable to complete the electronic forms, we have paper versions available upon check-in at the office. Please arrive approximately 20 minutes before your scheduled appointment time, and if needed, we will be happy to assist you with these.

For patients who would like to set up or modify auto-draft payments, please print and fill out the form below.

Once you complete this form, you may return it to our office via email to [email protected], or you may fax a copy to our office at 501-918-9942.

Insurance Update Form

If you are an existing patient and have an insurance change during your treatment, please use the link below to send that updated information to our office. Once you complete the form on the link and submit it back to us, we will verify any available benefits and file a claim with your new insurance.
