(Percentage covered and maximum amount) Typically, this is written in your policy like this…”50% coverage with a $1,000.00 lifetime maximum.” This means insurance will cover treatment with both of those criteria until the maximum is met. So if your plan covers 50% with a max of $1,000.00, but your treatment costs $3,000.00, then your insurance will only cover the $1,000.00 maximum, and leave the rest of the treatment fee for you to pay.

Generally, PPO plans have a larger network, and the office will know any fee schedules that may apply to the types of treatment they offer.
